WE ARE NO LONGER PARTICIPATING PROVIDERS WITH THE FOLLOWING INSURANCES: UNITED HEALTHCARE, COVENTRY, AETNA, AND ANTHEM. WE ARE PARTICIPATING PROVIDERS WITH CIGNA AND MEDICARE. THE REASON FOR THIS IS THAT WE TRY TO KEEP ADMINISTRATIVE AND OPERATING COSTS AS LOW AS POSSIBLE, AND PASS THE SAVINGS ON TO YOU.
Patients are seen by appointment only. The time for your appointment is reserved exclusively for you. Appointments may be made by calling 804-716-6267.
Registration forms may be completed on line or at your first visit. Please bring your insurance card and a photo ID with you for your first visit.
Payment for services is due at the time services are rendered. We accept cash, debit cards, and major credit cards. Please note, there is a $50 fee for checks not honored by your bank.
Payment for service is due and payable at the time the services are rendered. Although we do not participate with most insurances, we will be glad to give you an itemized statement for you to file with your insurance company for reimbursement if you have out-of-network benefits.
Bills are sent out for outstanding balances. Any balance unpaid after 90 days will be sent to a collection agency. If your account is sent to a collection agency, you will be responsible for the full balance due, an interest rate of 1.5% per month, and attorney fees and collection costs associated with the payment of the balance.
Cancellations require a minimum of 24 hours notice, and any appointment scheduled for a Monday (or the Tuesday after a Monday holiday) must be cancelled the Friday before. Appointments not cancelled within this time frame, or no-shows, will be charged a fee of up to the full cost of the session.
Patients are asked to bring all their medications with them for the first visit, including any over the counter medications, vitamins, and nutritional supplements.
Office telephone is answered from 9 AM to 6 PM Monday through Thursday and our office closed on Fridays. If you have a medical emergency during off-hours, please go to the nearest Emergency Department or call 911. Otherwise, please leave a message (on the confidential voice mail) and your call will be returned as soon as possible. We try to return all phone calls within 1-2 business days.
Patients who routinely cancel appointments will not have prescriptions renewed until they are seen in the office. Medical treatment of any illness requires the active involvement of the physician, and it is poor practice to prescribe medications indefinitely without seeing the patient at regular intervals.
If lab work is requested, please have this completed prior to the next appointment so we can discuss your results.
If you would like your medical records released to a third party, a release of information needs to be signed.

BRIEF FOLLOW UP VISIT $75 (<15 minutes)
INTERMEDIATE FOLLOW UP VISIT $100 (15-30 minutes)
EXTENDED FOLLOW UP VISIT $175 (31-45 minutes)
COMPLEX FOLLOW UP VISIT $200 (60 minutes)
URINE DRUG SCREEN $25
NO-SHOW FEE/LATE CANCELLATION FEE $50
COURT TESTIMONY $400/HOUR PLUS TRAVEL TIME
DOCUMENTS, FORMS AND REPORTS PRICED BY COMPLEXITY AND TIME
ALL FEES, EXCEPT THE INITIAL EVALUATION, ARE SUBJECT TO A SLIDING FEE SCALE. WE STRIVE TO MAKE MENTAL HEALTH TREATMENT AFFORDABLE, AND WE TRY TO CONSIDER OUR PATIENT'S FINANCIAL CIRCUMSTANCES.
